Can I use these dimensions as the card dimensions?

Add clearance first. The settings describe the finished front panel, and paper thickness, folds and glue reduce the usable internal space.

Why did the orientation change?

Portrait is used when the net fits with 5 mm clearance; otherwise landscape is tried. If neither fits, reduce the dimensions. Select the same orientation in the print dialog.

A simple pocket envelope: fold the side tabs inward, apply glue to the tabs, then fold the large bottom panel up. Fold the top flap down to close. Add clearance around your card; thick-paper allowances are not calculated.

Print at 100% / Actual size, with the matching paper and orientation, no margins, and browser headers and footers turned off. Choose Save as PDF in the print dialog for a PDF. Printer settings can override the document size.
